Application Instructions

The application period is from February 1 to March 15, 2020 during which time online and mail-in applications will be accepted. Mail-in applications must be postmarked between February 1 and March 15, 2020. Applicants are limited to one application per year. 

We advise the applicant to prepare their application on a separate sheet, e.g. in Word or on the mail-in application, to facilitate answering the short answer sections. Before submitting this form, please review your application thoroughly and make sure all required sections have been filled in completely and accurately. You will not have the opportunity to make any changes to your application after submitting this form.

You will receive an automatic email within approximately 30 minutes after the online application has been successfully submitted. If you did not receive a confirmation email, your application was not received.
